Who can be a patient at 85 HOPE?
You must be an adult resident (ages 18-64) of Wabash County, Indiana, low-income (based on household number and Federal Poverty Levels) and either uninsured or under-insured. We accept a higher household income than most people think!
How do I become a patient?
To become a client, please call or text: 260-274-0011.
The initial step will be to complete an intake appointment which includes going through your income information, consent forms, and health history.

How long before I can see a doctor?
As soon as you complete your intake appointment and are determined to be eligible for care, you will be scheduled to see the doctor as soon as possible. Your doctor appointment will usually occur either the same or following week as your intake appointment.
Do I have to have an appointment to see the doctor?
Yes, you must have an appointment to see the doctor. 85 HOPE functions similarly to any other doctor's office where patients are seen by appointment ONLY. 85 HOPE does not offer walk-in clinic services.
When does the doctor see patients?
At 85 HOPE Wabash, doctor appointments are scheduled on Thursday evenings from 5-8 pm.
Where are the clinics located?
85 HOPE Wabash: Lighthouse Crossings, 123 West Canal St., Wabash, Indiana, 46992. Patients should enter the clinic at the far left end entrance under the canopy.
How do I get my medicine?
85 HOPE has a small pharmacy on site at each clinic and will usually be able to provide any medication we prescribe free of charge. There are times when a patient will need get their medication at a different pharmacy. Financial assistance or vouchers can help cover the cost of a medication at an outside pharmacy.
Can I still use my own family doctor?
No. If you become an 85 HOPE client, 85 HOPE becomes your primary care provider (family doctor). You will no longer see your previous family doctor while you are a client at 85 HOPE.
Does 85 HOPE see kids?
No. Our patients must be ages 18-64.
What if I have a job? Can I still qualify to be a patient at 85 HOPE?
You may have a job and be a patient at 85 HOPE. Eligibility is based household number and income according to current Federal Poverty Levels.
What if I need to see a dentist or an eye doctor?
85 HOPE offers dental and optometry referrals for our regular medical patients ONLY. After you become a patient and see an 85 HOPE provider, we can refer you to a dentist or eye doctor depending upon your needs. 85 HOPE has resources to assist with and/or cover the cost for these referrals.
What if I need a PAP smear or mammogram?
85 HOPE hosts free Pap parties every other year (even years) in both Wabash and Manchester. This service is free to any woman in Wabash County ages 21-64. We also offer orders for free mammograms every October during Mammo Month to any woman in Wabash County ages 40-64.
What if I need lab work or X-rays done?
An 85 HOPE provider may order lab work or radiology testing as part of your care. All lab and radiology testing for 85 HOPE MUST be done at Parkview Wabash Hospital. Lab and radiology costs for 85 HOPE clients are free at Parkview Wabash Hospital.
