Who can apply?
You must be an adult resident (ages 18-64) of Wabash County, uninsured or under-insured, and have a household income at or below 200% of the federal poverty level.
How do I become a patient?
You will need to complete an intake appointment. You may schedule an intake appointment by calling, emailing, or visiting the clinic.
At your intake appointment, you will need to provide a driver's license, proof of residency (bill with your name, address, and the last month's date on it), and verification of your and your spouse's monthly income (last year's taxes, pay stubs, monthly benefits, or any other income).
**If you are unemployed, do not have a permanent address (homeless), or do not have access to the needed documentation, please call us or walk-in and we will be happy to assist you.**

How long before I can see a doctor?
As soon as you complete your intake appointment and are determined to be eligible, you will be scheduled to see the doctor. Your doctor appointment will usually occur either the same or following week.
Do I have to have an appointment?
Yes, you must have an appointment to see the doctor. Patients are seen by appointment only.
When does the doctor see patients?
At the Wabash site, doctor appointments occur on Thursday evenings from 5-8 pm.
At the Manchester site, doctor appointments occur on the 1st and 3rd Tuesday evenings from 5-8 pm.

Where are the clinics?
Wabash site: Friends Church, 3563 South State Road 13, Wabash, Indiana, 46992. Patients should enter the church at the SOUTH entrance (Door 9) and follow the signs.

Where do I get my medicine?
85 HOPE has a small pharmacy on site at each clinic and will usually be able to provide any medicine we prescribe for no charge. There are times when a patient will need get their medicine at a different pharmacy. Financial assistance is available for outside pharmacy charges.
Can I still use my own doctor?
No. If you are an 85 HOPE client, the clinic doctor is your primary care provider.

Do you see kids?
No. Our patients must be ages 18-64.
What if I have a job? Will I still qualify for help at the clinic?
You may have a job. Eligibility is based on the total household income and number of people in the household.
What if I need to see a dentist or an eye doctor?
After you become a patient and see an 85 HOPE doctor, we may refer you to a dentist or eye doctor depending upon your needs. 85 HOPE has resources to assist with cost for these referrals.
What if I need a PAP smear or mammogram?
In April of each year, we have two Pap Parties: one in North Manchester and one in Wabash. This service is free to any woman in Wabash County. We also offer orders for free mammograms at the Pap parties and during our Mammo Month event in October.
What if I need lab work or X-rays done?
An 85 HOPE provider may refer you to Parkview Wabash Hospital to have lab work or radiology tests done. Most lab work and x-rays costs are covered by either 85 HOPE or Parkview Wabash Hospital if you have a referral from an 85 HOPE provider.
